Introduction:

Exemestane is a steroidal suicidal aromatase inhibitor. Its structure and mode of action are very similar to Formestane, though its potent and effective characteristics are what truly stand out. Aromatase inhibitors are a class of drugs that combat estrogen by blocking the enzyme responsible for estrogen synthesis. Exemestane has been approved by the U.S. FDA for the treatment of breast cancer in women, especially those who have undergone tamoxifen therapy and are postmenopausal. Male bodybuilders and athletes often use this drug to mitigate the estrogenic side effects of aromatizable anabolic/androgenic steroids (this practice is not recognized within sports regulations), including gastrointestinal issues, weight gain, and water retention. In some cases, aromatase inhibitors can reduce body weight and help in sculpting muscles. Exemestane is one of the most effective aromatase inhibitors available. The most frequently cited data (from Aromasin's package insert) indicates that clinical studies have shown a reduction of estrogen levels by an average of 85% in women taking exemestane.

History:

Exemestane was developed by Pharmacia & Upjohn (Pharmacia) and received FDA approval at the end of 1999. The company introduced it to the market under the brand name Aromasin in early 2000. Although it has been proven that a low daily dose of 2.5 mg is effective for some patients, the company eventually developed the drug into a 25 mg tablet as the standard and common dosage. Subsequently, the company launched exemestane under the same brand name in other countries. Due to patent protections and market share expansion, Aromasin remains the only brand of exemestane and is the only one likely to achieve commercial success. Aromasin is currently available in over thirty countries, including Argentina, Australia, Austria, Belgium, Brazil, Canada, the Czech Republic, Chile, Denmark, Finland, France, Germany, Greece, Hong Kong, Hungary, Ireland, Israel, Italy, Malaysia, the Netherlands, Norway, New Zealand, the Philippines, Poland, Portugal, Russia, South Africa, Singapore, Spain, Sweden, Switzerland, Thailand, Turkey, the UK, the USA, and Venezuela.

How Supplied:

Exemestane is typically taken in 25 mg tablets.

Structural Characteristics:

Exemestane is an irreversible steroidal aromatase inhibitor. Its chemical formula is 6-methyl-enandrosta-1,4-diene-3,17-dione.

Side Effects:

Common side effects of using aromatase inhibitors include hot flashes, joint pain, decreased physical stamina, fatigue, mood instability, depression, high blood pressure, arm/leg swelling, and headaches. Aromatase inhibitors may also reduce mineral density in bones, which could lead to osteoporosis and fractures in people sensitive to bone issues. Some users may also experience gastrointestinal side effects, such as nausea and vomiting. Aromatase inhibitors are potentially harmful to fetal development and are therefore strictly advised against during pregnancy. For men using these drugs to reduce estrogenic effects from steroid therapy, aromatase inhibitors may increase the risk of cardiovascular diseases, as they reduce the beneficial effects of estrogens and the quality of cholesterol. Studies show that when aromatizable steroids like testosterone enanthate are taken with aromatase inhibitors, the development of beneficial HDL cholesterol levels is greatly suppressed. Since the estrogen receptor agonist/antagonist tamoxifen typically does not have the same anti-estrogenic (adverse) effect on cholesterol levels, it remains more popular among male fitness enthusiasts and athletes concerned with cardiovascular health in maintaining estrogen levels.

Usage Guidelines:

Exemestane is an FDA-approved medication used to treat early-stage breast cancer in postmenopausal women who are estrogen receptor-positive and have not responded well after 2-3 years of tamoxifen therapy. After tamoxifen is discontinued, an exemestane treatment should be initiated and continued for 2-3 years, followed by an additional 5 years of cumulative adjuvant drug therapy (i.e., combined use of tamoxifen and exemestane within the treatment course). The recommended dose of exemestane is one 25 mg tablet per day, taken after a meal. Male athletes and bodybuilders who use it to reduce estrogenic side effects from synthetic androgenic steroids, or for muscle sculpting purposes, typically take 12.5-25 mg of exemestane daily. In some cases, a dose of half a tablet every two days (i.e., 12.5 mg) is sufficient to prevent estrogenic side effects.
