Introduction:

Recombinant growth hormone is a synthetically manufactured form of human growth hormone (HGH). It is essentially a variant of the endogenous hGH protein, containing the same 191 amino acid sequence of human growth hormone, plus an additional amino acid, methionine. Thus, recombinant growth hormone is often described as recombinant human growth hormone. As a growth hormone medication, recombinant growth hormone supports the growth of bone, skeletal muscle, connective tissue, and internal organs. It also plays a role in the metabolism of proteins, carbohydrates, lipids, and minerals. Medically, recombinant growth hormone is used to treat growth disorders in children caused by a deficiency of endogenous growth hormone. When used as a long-term treatment after epiphyseal closure has stopped linear growth, this drug can have a significant positive effect on linear growth. Recombinant growth hormone is considered therapeutically equivalent to pituitary-produced growth hormone. As an HGH medication, recombinant growth hormone is highly valued by bodybuilders and athletes for its ability to promote fat loss and the growth of muscle and connective tissue.

Despite being considered equivalent to human growth hormone, recombinant growth hormone is not a natural protein in the body. This may increase the likelihood of developing growth hormone antibodies during treatment. Antibodies act by binding to growth hormone molecules, affecting their ability to function. In one clinical study, two-thirds (2/3) of children treated with recombinant growth hormone developed growth hormone antibodies after one year. In a similar survey on growth hormone management, only one out of seven patients developed serum growth hormone antibodies. Notably, in both studies, the antibody response was not robust and did not appear to significantly reduce the drug's therapeutic effectiveness. The reduced effect (determined by antibody levels) occurred in a very small percentage (less than 1%) of patients. Nevertheless, the original 191 amino acid growth hormone is considered more worthwhile to use.

History:

Recombinant growth hormone was approved for sale in the United States in 1985. It was the world's first synthetic growth hormone drug, produced through a manufacturing process known as inclusion body technology. This technology involves inserting the DNA encoding human growth hormone genes into E. coli bacteria to assemble and synthesize pure protein. Before the advent of synthetic growth hormone, human growth hormone was made from natural proteins extracted from human cadavers. In 1985, the use of biologic or cadaver-derived growth hormone was banned in the United States due to a rare neurological disease epidemic among patients. Recombinant growth hormone was approved for sale in the same year, giving Genentech a short-term monopoly on the growth hormone market. However, within a few years, other biotech companies began selling a growth hormone identical to the endogenous protein, known as somatropin. Although recombinant growth hormone is still available in many markets, including the United States, the distribution of growth hormone is more widespread.

Available Specifications:

Recombinant growth hormone is most commonly available in multi-dose vials containing white lyophilized powder that needs to be reconstituted with sterile or bacteriostatic water before use. Each vial's dosage can vary from 1 milligram to 10 milligrams.

Structural Characteristics:

Recombinant growth hormone is a peptide manufactured through recombinant DNA technology. It consists of 192 amino acid residues, with a molecular weight of 22256.

Storage:

Do not freeze. Keep refrigerated (2º to 8ºC, 35º to 46ºF) before and after reconstitution.

Side Effects (General):

The most commonly reported adverse reactions to recombinant growth hormone therapy include carpal tunnel syndrome, nevus growth (moles and birthmarks), gynecomastia in males, and pancreatitis. It's important to note that the side effects of recombinant growth hormone generally resemble those of growth hormone therapy. Misuse of recombinant growth hormone can lead to diabetes, acromegaly (thickening of bones, most visibly in the forehead, hands, jaw, and elbows), and enlargement of internal organs. Due to the growth-promoting effects of human growth hormone, this drug should not be used by individuals with active or recurrent cancer.

Side Effects (Impaired Glucose Tolerance):

Recombinant growth hormone can reduce the body's insulin sensitivity, thereby raising blood sugar levels. This may occur in individuals without diabetes or glucose intolerance.

Side Effects (Injection Site):

Subcutaneous injection of recombinant growth hormone may cause redness, itching, or lumps at the injection site. It can also lead to local loss of fat tissue, possibly caused by repeated injections at the same site.

Usage Guidelines:

Recombinant growth hormone is administered via subcutaneous or intramuscular injection. 1 milligram of recombinant growth hormone is approximately equivalent to 3 International Units (3IU). When used to treat children with growth disorders due to growth hormone deficiency, the dose is administered at 0.04 milligrams per kilogram of body weight daily. For an individual weighing approximately 180 pounds, this equates to about 10IU (International Units) per day. Over time, the long-term maintenance dose, assessed after evaluating the patient's IGF-1 levels and clinical response, may be significantly less than 10IU (International Units).
